Printable Application form Instructions.
- Only one applicant per application form.
- Full name of applicant requesting WAM membership.
- Date of birth of applicant requesting WAM membership.
- AMA number of applicant requesting WAM membership.
- Address of senior applicant applying for WAM membership or address
of guardian of junior applicant applying for WAM membership.
- Home phone number of senior applicant applying for membership or
home phone number of guardian of junior applicant applying
for WAM membership.
- When a junior applicant is applying for WAM
membership the junior applicant must sign the application at the bottom
signifying that he or she have been advised of
the waiver and release of liability statement on the form
by his or her parent or guardian. And the parent or guardian
of the applicant must also sign the application at the bottom.
- To Have your Printable Application Print properly on one
sheet of paper, Select the Printable Application page and
verify that the following settings have been set: Chose
File then Page Setup and verify the following settings
have been set, Paper Size = 81/2 x 11in, Source = Auto
Sheet Feeder, Orientation = Portrait, Margins (inches) =
Left 0.25, Right 0.25, Top 0.166, Bottom 0.575. Next
Chose View then Text Size and verify Text Size is set to
Smaller. Hopefully with the above settings set, your
Computer/Printer system will print out the Application
properly on one page, if not the WAM Secretary shall
accept the Application printed on two pages and we thank
you for helping us to limit the amount of paper we are
required to store.
Print the Application Form, Completely fill out the form, Prior
to mailing the form With your Money Order or Check
enclosed, Verify that the form has been Signed by the Proper Persons
in the proper spaces at the bottom of the form, Enclose a copy of the AMA Membership
Card for each pilot requesting WAM membership and a (LETTER SIZE) self addressed
stamped envelope, so your WAM Membership card can be mailed to you.
